We know our referral form asks for a lot. You might wonder, “Can’t I just give you my name and number and chat first?”

We understand that hesitation. However, in the current NDIS system (PACE), accuracy is everything. Without the right details upfront, the system blocks us from connecting with you.

Here is why we need specific details to complete your NDIS intake process:

1. Your Full Legal Name & NDIS Number

  • The Reason: To find you in the system. The NDIS portal is strict. If your legal name is “Robert” but you write “Bob,” the system rejects our request.

  • The PACE Factor: For Agency Managed participants, we need these details to trigger the “Provider Endorsement” request. Without this, the NDIS cannot pay us, and we cannot start working.

2. Your Plan & Goals

  • The Reason: Matching magic. We don’t just assign you to the “next available” person. We review your goals to match you with the right expertise.

3. Your Current Support Network

  • The Reason: To respect your team. If you already have great providers, we want to complement them, not step on their toes.

Your Privacy is Non-Negotiable

We treat your data like it is our own.

If you make an enquiry but decide not to proceed with CoAbility, we delete your personal details. We do not hoard data. We follow the strict NDIS Practice Standards regarding privacy.

Your NDIS Intake Process Timeline

Once you submit your form, what happens next?

  • Day 1: The Review Our Intake Team reviews your information. We check your funding to ensure you have the budget for Support Coordination.

  • Day 2: The Match We look at our team’s capacity and skills to find your best fit.

  • Day 3: The Introduction You receive an introduction to your new Support Coordinator.
